We don't manage Direct Debit forms from within Westpac. They are prepared and managed by companies such as your phone or energy provider.

To set up a new Direct Debit arrangement with a particular company, please ask them for a copy of their Direct Debit form. Many companies have the form available on their website.

To complete the form you'll need the customer number shown on your bill, your bank account number, and our main postal address:

(The form asks for the branch postal address, but our main address is actually more useful).

Once you've completed the form, return it to the issuing company for processing.
